What You Will Need to Get Started

When you and your doctor choose to begin treatment with OXLUMOTM (lumasiran), the first step is to complete the Start Form together.

Before you begin this process with your doctor, please take a few minutes to review the types of information you will need to fill out for your sections of the Start Form, especially if you have additional information you need to provide for a loved one:

Personal Information

Personal Information

This includes basic information like your name, date of birth, current mailing address, email, and phone number.

Primary Insurance Information

This includes information found on your insurance card, if you have one. You will need your plan name, policy number, and group number, so be sure to have your insurance card available.

Physician Information

In order to get started with Alnylam Assist®, your doctor is required to initiate your enrollment in the program and provide their information.

How to Begin the Start Form

The first step towards getting started with the program is to complete the Start Form with your doctor.

Your doctor has options on how to do this. Doctors will either 1) complete the form with you present, or 2) begin the form by filling out their physician information, then share with you via email to complete your portion.

Then, your doctor will submit the form to Alnylam Assist®. Upon receipt of the Start Form, your dedicated Alnylam Case Manager will reach out to you and your doctor within 1-2 business days.

IMPORTANT SAFETY INFORMATION

The most common side effect of OXLUMOTM (lumasiran) is injection site reaction (redness, pain, itching, and swelling at the site of injection). These are not all the possible side effects of OXLUMO. Talk to your doctor about side effects that you experience. You are encouraged to report negative side effects of prescription drugs to the FDA. Visit www.fda.gov/medwatch, or call 1-800-FDA-1088.

OXLUMO has not been studied in pregnant or breastfeeding women. Talk to your doctor about the risk of taking OXLUMO if you are pregnant, plan to become pregnant, are breastfeeding, or plan to breastfeed.

WHAT IS OXLUMO?

OXLUMO is a prescription medicine for the treatment of primary hyperoxaluria type 1 (PH1) to lower oxalate in urine in children and adults.
